Objective

Rewrite docs/EVIDENCE.md as the reference of the finished model, with the pipeline drawn in mermaid and a case named on every row of the matrix, and hold it to the tree with a case; cite ADR 0068, 0080 and 0087 structurally once they have landed.

Why

docs/EVIDENCE.md is shared by section while the work runs, so it can describe the finished model only after the slices land, and a reference that names what the code no longer declares is a stale projection.

Current state

docs/EVIDENCE.md describes the ledger and its states; its sections are edited by I1948 and I1949 while the work runs.

Desired state

The reference names every command, capability, state and file the code declares for evidence, draws the pipeline in mermaid, and a case fails when it names something the tree does not have or omits something it does.
